Billing

At Functional Physical Therapy, Inc., we strive to make the billing process of your physical therapy as clear and efficient as possible.  As a new patient, we will copy your ID and insurance card.  Then, as a courtesy to you, we will contact your insurance comapny and verify your benifits for physical therapy treatments.  We will inform you of any deductibles, co-pays or co-insurance, as well as your visit limits per your individual plan.

Please not that your coverage is between you and your insurance carrier.  While we do verify your benefits, it is not a guarantee of payment or coverage.  All deductibles and co-pays/co-insurances are set by your insurance company for you particular plan, not by Functional Physical Therapy, Inc.

After we have submitted the billing to your insurance company, we will receive an Explanation of Benifits that will inform us of your responsibility.  We will mail out patient statements on a monthly basis.  This is dependent on the timing and efficiency of you insurance company.

Functional Physical Therapy offers several payment plans for our patients to choose from.  We value our patients and understand certain financial situations, and are willing to work with you. Please contact our billing department to discuss a payment option that works best for you.
